- We are OUT OF NETWORK for ALL Medicare Advantage plans.
- We are OUT OF NETWORK for ALL private insurance plans.
We are no longer submitting any claims for Medicare Advantage plans. Medicare Advantage patients will be charged Medicare limiting rates. We will provide a superbill, and you will be responsible for submitting for re-imbursement.
Depending on the type of Medicare Advantage plan you may be required to see a provider in-network. Your plan may also require a referral. Find out whether your plan allows you to use providers outside of its network by checking your coverage or contacting your plan. Note, if your Medicare Advantage plan does allow you to seek treatment from providers outside of the plan network, you are likely to have a higher co-insurance.

Insurance
- We accept Medicare Part B and Worker’s Compensation cases.
- We are an out-of-network provider and do not participate in any commercial insurance plans. Payments are due on the date of service; If requested, superbills are provided for patients to submit to their insurance for reimbursement.
- Please note, Medicare Part B must be your primary insurance.
- We do not accept Medicare Advantage plans and payments will be due on the date of service.
- Please note, if you are a Worker’s Compensation case, we will not schedule you until we receive an official “Letter of Authorization.”
- A Good Faith Estimate will be provided if requested. We will ask for your insurance information first.
